How to Fill the SSB Form: The 12 Mistakes That Get Forms Rejected
A wrong date of birth or an unsigned medical slip can cost you a recommended candidature. A complete checklist before you submit.

1. Date of birth mismatch
Your SSB form, your Aadhaar, your 10th marksheet and your CDS/NDA application must all carry the same date of birth. A single mismatch triggers a verification loop that can cost you the entire attempt.
2. Missing signatures
Every page needs a signature in the correct column. Unsigned forms are returned, not corrected.
3. Photograph dimensions
One recent passport-size photograph, clear face, plain background, no sunglasses, no selfie.
4. Name spelling consistency
"Hitender" on the form and "Hitender" on the marksheet. Do not "correct" your name in one place and forget the other.
5. Address mismatch
If the address on the form differs from your current address, attach a proof. Do not leave the officer to guess.
6. Blank boxes
Do not leave a box blank. Write "NIL". A blank box is read as an omission.
7. Medical report timing
The medical examination must be completed within the validity window. A report older than the prescribed period is invalid.
8. Missing CBE / CIE details
Centre for Military Evaluation and Centre for Intelligence Evaluation entries must be filled in exactly as instructed in your official SSB call letter.
9. Wrong category ticked
SC/ST/OBC/NCL, General, and ex-serveman categories change the shortlisting. Verify before submission.
10. Illegible entries
Printed in black ballpoint. Sloppy handwriting invites rejection at the document check counter before a single question is asked.
11. Missing passport photos
Carry four extra photographs on Centre Day. Always.
12. Not carrying originals
Originals plus two photocopies of every document. A photocopy queue at the document counter has ended more hopes than bad interviews.
